Temporary Disability	Permanent Disability
Receive 1 or 2 placards	Receive 2 placards or 1 placard and a set of disability plates
Valid for 6 months	Valid for 4 years
Cost $5 each	No Cost for placards/ Plates a small fee applies

Please note: Must be a Williamson County Resident.

Bring the following documents to one of our locations:

Completed Persons with Disabilities Parking Placard and/or License Plate (VTR-214)
An original prescription from a physician or a notarized physician's signature on the back of form VTR-214
Valid TX DL or ID, unless active duty U.S. Military or non-resident seeking treatment in Texas

Applications may take 10 to 15 business days to process enclose the following documents:

Completed Persons with Disabilities Parking Placard and/or License Plate (VTR-214)
An original prescription from a physician or a notarized physician's signature on the back of form VTR-214
Valid TX DL or ID, unless active duty U.S. Military or non-resident seeking treatment in Texas
If applying for temporary disability placards please include a check or money order made payable to Larry Gaddes, CTA
- Payments made by check may be converted to an electronic funds transfer.
